Contact LTRG Office for $100 Gift Card for Ray's for Survivors

Survivors are invited to come by the LTRG offices above the Weed Library at 150 Alamo to help us update our contact information for you and to receive a $100 Ray’s card. This includes renters and homeowners who lost their primary residence.
This is a project funded by the Shasta Regional Community Disaster Relief Fund to Great Northern Services in support of the LTRG case management request.
In addition to making sure that we have correct contact information for Boles Fire Survivors, we want to ask about your current status and future needs to help us determine any needs for assistance, including volunteer help with rebuilding and allocation of other resources. We need to know who has built and who is planning to rebuild and where they are in the reconstruction process. We want to stay connected, even if you have moved, we want to hear from you.
Case Managers, including three trained volunteers, will be present to interview each person privately.
We will be interviewing Boles Fire Survivors at the following times:
Monday, December 14, 2015 from 8 AM—1 PM Wednesday, December 16, 2015 from 12 PM—6 PM Thursday, December 17, 2015 from 4 PM—8 PM
We will continue to give out the Ray’s cards after the above dates by appointment only so that we can be sure that we have someone to help you.
There is a flight of stairs up to the offices. If this is difficult for you, please call and I will set up an appointment for you at an accessible location.
The form is listed under the "Documents" button on the left side of the home page. You can fill it out in advance or at our office.
